Waiver, Release of Liability & Informed Consent

THIS IS A LEGALLY BINDING DOCUMENT. READ IT CAREFULLY BEFORE SIGNING.

1. The Activities and Their Risks

Metric Strength provides supervised strength and cardiovascular training using specialized equipment: motorized adaptive resistance equipment and high-intensity interval cardio bikes.

Strength sessions: the machines use motorized resistance that pushes back against me. I agree to follow my trainer's safety instructions (including never locking my knees or elbows against the resistance) and I understand that maximal-intensity resistance training carries risks including joint injury, significant muscle soreness, and, rarely, rhabdomyolysis (rapid muscle breakdown).

Cardio sessions: the bike uses short maximal-intensity sprints that rapidly elevate heart rate, which places significant demand on my cardiovascular system and carries risks including dizziness, fainting, and adverse cardiac events.

I am participating voluntarily, and I knowingly assume these risks and the other inherent risks of physical training, whether or not listed here.

2. Release of Liability (Ordinary Negligence)

In exchange for being permitted to participate, I release and discharge Metric Strength LLC, its owners, officers, employees, agents, and landlords (the "Released Parties") from all claims, demands, and causes of action arising out of any loss, damage, or injury, including death, sustained by me in connection with the activities or the premises, including claims caused by the ordinary negligence of the Released Parties.

This release does not extend to gross negligence, reckless misconduct, or intentional wrongdoing, and it does not waive any right that cannot be waived under Ohio law.

3. My Responsibilities

I agree to follow my trainer's instructions and the studio's safety rules, and to ask before using any equipment I have not been instructed on.

I have answered the health-readiness (PAR-Q+) questions honestly and completely. I agree to follow any restrictions or clearance requirements identified through that process, and to tell Metric Strength about material changes to my health.

4. Signing

I confirm that I am at least 18 years old and am signing for myself.

I HAVE READ THIS WAIVER, FULLY UNDERSTAND THAT IT RELEASES CLAIMS INCLUDING FOR ORDINARY NEGLIGENCE, AND SIGN IT FREELY AND VOLUNTARILY.
